Monday June 5th is our annual Athletic Physicals on campus! As a reminder all the proceeds go to our Athletic Training dept which goes directly back to our student athletes care! See attached flyer or description below for instructions.
1. Download and Print the Physical Form.
2. Complete “History” side of the Physical From (parent and athlete are required to sign the bottom of the form).
3. Bring $35 Cash or Check (make check payable to "Huntington Beach High School").
